π Forms Bot β User Guide
Everything you need to open support tickets and submit applications on our server.
Overview
The Forms bot handles two things on this server:
π« Support Tickets
Need help from staff? Open a private ticket channel. Only you and staff can see it. When your issue is resolved, a staff member closes it.
π Applications
Applying for a role or position on the server? Click Apply and answer questions in your DMs. Staff review your answers privately.
Everything is done through buttons and menus β you don't need to type any commands.
Opening a Ticket
Find the ticket panel channel on the server. It looks like this:

Click "π« Open Ticket"
A small menu will appear, visible only to you, asking you to choose a category. -
Select a category from the dropdown
Categories are topics like "General Support", "Bug Report", etc. Pick the one that best describes your issue. -
Your ticket channel is created
A new private channel appears in the server sidebar. Head there now.
You have 2 minutes to pick a category after clicking the button. If the menu disappears before you choose, just click the button again.
Inside Your Ticket
Once your ticket channel is created, you'll see a welcome message:
Just type your message describing what you need help with. Staff will reply as soon as they're available.
Only you and staff can see this channel. Feel free to share details, screenshots, or anything else that helps explain your issue.
You can have up to a set number of tickets open at the same time. If you try to open more than the limit, you'll get a message telling you to wait for your existing tickets to be resolved first.
Closing a Ticket
Only staff members can close tickets using the button. When a ticket is closed:
- A transcript of the conversation is saved.
- The transcript is sent to you via DM (if your DMs are open β see DM Issues).
- The ticket channel is deleted.
If you need a copy of your ticket conversation, make sure your DMs are open before staff closes the ticket.
Submitting an Application
Find the channel where the application is posted. It will have an Apply button like this:
Before you click Apply, make sure your DMs are open. The bot sends your questions through direct message. If your DMs are closed, you won't be able to apply. See how to enable DMs.

Click "π Apply"
You'll see a confirmation that your first question has been sent to your DMs. -
Check your Direct Messages
Open your DMs and look for a message from the bot. It will say something like:

Type your answer and send it
Reply directly in the DM. The bot will confirm your answer was saved and send the next question. -
Repeat for all questions
Keep answering each question in the DM. After the last one, you'll get a confirmation that your application was submitted.
Answering Questions
After each answer, the bot confirms it was saved and immediately sends the next question:
Important β don't leave your DMs for too long. You have time to answer each question, but if you go completely idle, the application may get stuck. There is no strict timer while you're actively replying β just don't walk away mid-application for hours. If this happens, see Timed Out.
A few things to keep in mind:
- Answer in the DM conversation with the bot, not in any server channel.
- Each answer is saved as soon as you send it β you can't go back to edit a previous answer.
- You can only have one application in progress at a time. Finish your current one before starting another.
After You Apply
Once all questions are answered, you'll see:
Staff will review your answers privately. You'll receive a DM when a decision is made:
If you're denied, there's a waiting period before you can apply again. The bot will tell you how long if you try to click Apply before the cooldown is up.
Troubleshooting
Something went wrong? Find your issue below.
DM Issues
Your Discord privacy settings are blocking the bot from sending you a DM. This is the most common issue with applications.
- Right-click the server icon in the left sidebar.
- Click Privacy Settings.
- Turn on "Allow direct messages from server members".
- Go back and click the Apply button again.
- Tap the server name at the top.
- Tap Privacy Settings.
- Turn on "Allow direct messages".
- Go back and tap the Apply button again.
You only need to allow DMs from this server. You don't have to allow DMs from all servers. Once you're done with the application you can turn it off again.
The bot sent the message, but it may be hidden or you're looking in the wrong place.
- In Discord, click the speech bubble icon (Direct Messages) at the top of the left sidebar.
- Look for Forms Bot in your DM list β it may be scrolled up.
- If DMs are enabled but you still don't see it, try clicking Apply again. If it says "you already have an application in progress", the DM was sent β just scroll up in your DM list to find it.
The bot sends the decision via DM. If you didn't receive it, your DMs were likely closed when staff made the decision.
- Enable DMs (see above) and ask a staff member to let you know the outcome.
- If you were approved, check whether your roles on the server have changed.
Timed Out
Many of the bot's menus have a time limit. If you wait too long, they expire and you'll need to start again. Here's what each timeout means and what to do.
The category selection menu has a 2-minute timeout. If you didn't pick a category in time, it disappeared.
Simply click the button again. No data was lost.
The application DM flow doesn't have a strict per-question timer while you're actively replying. However, if you left the DM conversation idle for a very long time (many hours) without sending anything, the internal session may have expired.
You'll know something went wrong if you reply in the DM and nothing happens β the bot doesn't ask the next question and doesn't confirm anything.
- Go back to the application channel and try clicking Apply again.
- If it says "You already have an application in progress", contact a staff member β they can reset your application state so you can start fresh.
- Note: Any answers you already sent are not saved if the session expired. You'll need to answer all questions again from the beginning.
To avoid this: once you start an application, try to complete it in one sitting. The questions aren't timed while you're actively typing, but don't leave Discord for hours mid-application.
Ticket Problems
Ticket channels are deleted when a staff member closes the ticket. This is normal.
- The bot should have sent you a transcript via DM when the ticket was closed.
- If you didn't receive the transcript, your DMs were likely closed. Contact staff if you need the information.
- If your issue isn't resolved, you can open a new ticket.
The Discord category where tickets are created was deleted or moved. The bot can't create new ticket channels until this is fixed.
This requires a staff member or admin to fix. Let them know so they can re-run /forms setup or update the settings.
Setup hasn't been completed, or the ticket categories were deleted from the settings.
A staff member or admin needs to complete setup or re-add categories via /forms settings.
Application Problems
You were previously denied and there's a waiting period before you can apply again.
- Wait until the cooldown expires, then click Apply again.
- Use the waiting time to work on your application β review any feedback staff gave you in the denial DM.
You clicked Apply but already have an unfinished application from a previous attempt.
- Check your DMs with the bot β there may be an unanswered question waiting for you.
- If you can't find the DM or the bot isn't responding there, contact a staff member to clear your application state.
The application template was deleted by staff after you clicked Apply.
Contact staff to find out if the application is still available or has been replaced.
Frequently Asked Questions
Can I edit my answers after I send them?
No. Each answer is recorded the moment you send it in the DM. If you made a mistake, contact staff after submitting and let them know.
Can staff see my ticket before I type anything?
Yes. The channel is created as soon as you select a category, and staff have access to it immediately. Type your question as soon as possible.
Will I lose my application progress if Discord crashes?
Each answer is saved immediately after you send it. If Discord crashes mid-application, your answers up to that point are safe. Just open your DMs with the bot and answer the next question when you're back.
Why did the bot send me "Starting your applicationβ¦" but then nothing else?
This is a test message the bot sends to confirm your DMs are open. The real first question comes right after. If you only received "Starting your applicationβ¦" and then nothing, try scrolling up in the DM β the question may have loaded just above it.
Can I apply to multiple things at the same time?
No. You can only have one application in progress at a time. Finish your current one first, then apply to the other.
What happens if staff never responds to my ticket?
Staff manage tickets as quickly as they can. If your ticket seems to have been missed, you can mention it in a public server channel to get their attention (without repeating the details β just note that your ticket hasn't had a reply).
I got approved β when do I get my role?
The role is added automatically the moment staff clicks Approve. If you click on your profile and don't see the role, try leaving and rejoining the server. If it's still missing, contact staff.
